- The copyrighted work you say has been infringed.
- The MelodyBin link(s) to the material you want removed.

When we receive a valid notice, we'll promptly remove or disable access to the material and tell the uploader. Note: knowingly misrepresenting that material is infringing can make you liable for damages (§ 512(f)).
Counter-notice
If you think your track was removed by mistake or misidentified, send our agent:

- A statement, under penalty of perjury, that you believe in good faith it was removed because of a mistake or misidentification.
- Your name, address, and phone number, and a statement that you consent to the jurisdiction of the federal district court for your address (or, if you're outside the U.S., any district where MelodyBin can be found), and that you'll accept service of process from the person who sent the original notice.
We'll forward your counter-notice to the person who sent the original notice. If they don't tell us within 10 business days that they've filed a lawsuit, we may restore the material within 10 to 14 business days.

We give one strike for each valid notice that isn't successfully disputed. If an account gets {3} strikes within 12 months, we close it. We may also close an account sooner for clear or deliberate infringement.
